Scheduling appointments

Routine appointments are scheduled for weekdays between 7:45 a.m. and 4:30 p.m.  When calling for an appointment, be sure to state the reason for the appointment, if you are a contact lens wearer, and any specific concerns that you may have.

You will be mailed forms to fill out regarding your past medical history and demographic information including insurance.  Bring these and your insurance cards with you to your appointment.
